Please read before proceeding

  1. You can submit a request for Removal Form only after the expiration of the minimum one year or five years self-exclusion period you chose on your Request for Voluntary Exclusion from casino and simulcasting activities, racetrack sports wagering activities and internet gaming and sports wagering activities form. If you chose the lifetime exclusion option, your name cannot be removed from the Self-Exclusion List.

  2. By acknowledging and submitting this online application, you are removing your name from the Self-Exclusion List for casino and simulcasting activities, racetrack sports wagering activities, and/or internet gaming and sports wagering activities and authorizing the Division of Gaming Enforcement (Division) to permit all New Jersey casino and simulcasting facilities, racetrack sports wagering facilities and internet gaming and sports wagering platform providers to reinstate your gaming privileges. However, you may continue to be subject to other Responsible Gaming Programs enacted by casino and simulcasting facilities, racetrack sports wagering facilities and internet gaming and sports wagering platform providers. The Waiver of liability you signed in connection with the submission of your Request for Voluntary Exclusion Form remains in effect.

  3. The Division has five business days from the receipt of your Removal Form to delete your name from the Self-Exclusion List and notify the Atlantic City Casinos, racetrack sports wagering facilities and internet gaming and sports wagering platform providers of such removal. Consequently, you may be denied gaming privileges from the time you submit your Removal Form until the casinos update their records.

  4. In accordance with Section 7 of the Privacy Act, 5 U.S.C. 552a, disclosure of your social security number to the Division is voluntary. Failure to disclose your social security number is not grounds for denial of your request for removal from the Self-Exclusion List. The request for your social security number is made pursuant to the Casino Control Act, N.J.S.A. 5:12-1, et seq. If provided, your social security number will be used by the Division to obtain and verify information contained in this request. The absence of a social security number may delay the final determination of your request. Voluntary submission of your Social Security Number is required if you are requesting to be removed from the Self-Exclusion List through the online portal. If you do not want to disclose your social security number, you can contact the Division of Gaming Enforcement at 1-833-788-4DGE to request a video conference or in- person conference at either our Atlantic City or Trenton locations.

  5. For the complete rules governing the Internet Gaming Self-Exclusion program, see the Casino Control Act at N.J.S.A. 5:12-71.2 and 71.3, as well as the regulations set forth at N.J.A.C. 13:69G-2.1 through 2.5. Copies of these rules are available upon request from Division personnel and are also available on the Division's website. Rules may be changed without prior notice to Self-Exclusion program participants.
